    Form W-2 -- When preparing the Forms W-2, use your 15 digit Wisconsin tax account number:

    • Complete Form WT2 (Paper W2 Transmittal Sheet)
    • Attach the form to the Forms W-2

  4. I am using a private delivery service to send my return. What mailing address should I use?

    Private delivery services cannot deliver items to P.O. boxes. If you are sending tax return information to the department using a delivery service, you must use the department's street address:
