How to Order Your Library's Wisconsin Tax Forms

You should order your library's Wisconsin income tax forms by using our library forms order system. You can access the forms order system here: FORMS ORDER SYSTEM. You can also access this system on our website at revenue.wi.gov.

• Click the Governments tab
• Click Libraries
• In the Order Forms box, select Forms Order System

Step 1: Enter the following information in the Library Screen.

• Phone Number
• Contact Name
• Library Name
• Shipping Address
• City State Zip

Step 2: Click on NEXT, which will bring you to the Selection Screen. Enter the number of forms needed in the box in front of each form type.

Step 3: Click on NEXT, which will bring you to the Review Screen.

• If your order is not correct, click on PREVIOUS to return to the LIBRARY or SELECTION screen to correct information.
• If your order is correct and you want to print a copy of your order, RIGHT CLICK and select PRINT.
• Click on PLACE ORDER to complete your order.
