How to Change Your Address, Name, or Taxpayer Identification Number (En Español)

You can notify the Department of Revenue by:
- Mailing Form P-706, Taxpayer Information Change Request, to the department. You may obtain this form by:
- Downloading the form from the department's website.
- Requesting the form by calling our Forms Request Office at (608) 266-1961.
- Including the necessary information in a letter to:

Include your name, tax account number or last four digits of your social security number, and address in all correspondence. Also include information specific to your request, including:
- Your name
- Your spouse's name
- Change in your name or your spouse's name
- Your old address
- Your new address
- County of your new residence
- City, village, or township of your new residence
- Incorrect social security number
- Correct social security number
Include your daytime telephone number in case we have questions. Include a copy of your social security card for a change in the social security number.
If you are changing information for anyone other than yourself, Form A-222, Power of Attorney, must be provided.

A business can notify the Department of Revenue in one of the following ways:
Address Change
- Indicate the change on the next business tax return filed with the department.
- Write to the Wisconsin Department of Revenue, P.O. Box 8946, Madison, WI 53708-8946. Include the name, federal identification number, old address and new address.

- Submit an address change request through My Tax Account.
Name Change & FEIN Change
- Indicate the change on the next corporation or partnership return filed with the department.
- Write to the Wisconsin Department of Revenue, P.O. Box 8908, Madison, WI 53708-8908. Include the old and new name, old and new federal identification number, and address.
- My Tax Account users can submit requests to change their "doing business as" name or add their FEIN to their account if the department does not already have that number on record.
Note: A corporation, limited liability company filing as a corporation or partnership, or limited partnership that is required to register with the Wisconsin Department of Financial Institutions (DFI) may change its name by writing to the Department of Financial Institutions, P.O. Box 7846, Madison, WI 53707-7846 or calling (608) 261-7577.

You may mail, fax or email the change(s) to DOR:

Be sure to include:
- The correct legal name of the governmental unit and the County and Municipality and/or Special District Code (if applicable).
- Full name, title (position), mailing address, phone number and email address of the new official.
- Effective date.
- Name of official being replaced.
