Form W-RA Attachments Data File Transmission

Individuals, and tax practitioners filing on behalf of individuals, required to send Form W-RA and its attachments to the Wisconsin Department of Revenue (DOR) may now transmit this data in an electronic file over the Internet.

You may use this application if you: Instructions for sending your file:
  • Filed your 2012 income tax return and/or homestead credit claim electronically.
  • Are required to send attachments per the Form W-RA instructions.
  • Are required to send in attachments for one or multiple individuals.

Ready to Transfer

  1. We can only accept .ZIP files containing PDF, TIF, JPG or BMP files.
  2. We can only accept files less than 50MB in size. Files larger than 50MB should be divided into two or more .ZIP files.
  3. Form W-RA and each attached file must be named in the following format:

Taxpayer's 9-digit social security number, immediately followed by 2012 (the tax year), immediately followed by a number; i.e., 1, 2, 3, etc.

Example: If the taxpayer's social security number is 123-45-6789 and the tax year is 2012, the attachment file names would be…

  1. Each attachment file name must be in the format described above and then the attachment files must be zipped together as follows:

These attachment files…

  • 12345678920121.PDF
  • 12345678920122.TIF
  • 12345678920123.BMP

Would be zipped together to create a .ZIP file called Taxpayer1.ZIP.

CAUTION: Do not put these attachment files into a folder and then put the folder in the zipped file. They will not attach to the appropriate account.

Would be zipped together to create a .ZIP file called Taxpayers.ZIP.

CAUTION: Do not put these attachment files into a folder and then put the folder into a zipped file. They will not attach to the appropriate account.

NOTE: If you do not follow this format, the file will not be attached to the appropriate account. As a result, DOR has no record of the documents. DOR will take no action on the homestead credit claim until the claimant resubmits the Form W-RA and attachments.

  1. Click on the Ready To Transfer link to begin the transfer process.
  2. Know where the Form W-RA and attachments are on your PC or Network. Make sure the documents being transmitted are clear and readable.
  3. Make sure that the radio button for "Is this a test file": is checked "No."
  4. On the "Select the File" line, click on the Browse button to locate the required attachments for the Form W-RA Attachments data file on your PC or Network.
  5. Click the “Transfer” button to submit your file.
