The Wisconsin Department of Revenue (DOR) offers and accepts electronic funds transfer (EFT) as a method of making one-time payments or monthly payments to reduce your balance owed.
- File one-time payment: For individuals, go to Online Services to Pay online. For businesses, logon to My Tax Account.
- File monthly payment request: The forms can be found here to request a payment plan (Form A-771) and to authorize EFT. Fill out the form and submit through our secure online process.
- File on paper: The forms can be found here to request a payment plan (form A-771) and to authorize EFT. Print the forms. Fill out, sign, and mail to Central Compliance Section, Wisconsin Department of Revenue, PO Box 8901, Madison, WI 53708-8901.
- To request a form: Send an email or letter to Central Compliance Section, Wisconsin Department of Revenue, PO Box 8901, Madison, WI 53708-8901.
Once DOR approves your payment plan, you can choose to have the payment automatically withdrawn from your bank account and credited to your balance. In some situations, we will require that your payments be made through EFT. To use EFT, you must complete an authorization form first. You also have the option of a one-time withdrawal for the entire balance on your account.
Payment plan terms require that you file and pay all tax returns on time and make the expected payments. As long as you honor these terms, we will not take other collection action. However, we will collect any refunds or payments due to you from the federal government, Wisconsin and other states and may issue a tax warrant to secure the debt.
